OSHA Silica Enforcement Tightens in 2026 — Why On-Site Chest X-Ray and Spirometry Belong on the Jobsite

OSHA's respirable crystalline silica standard for construction (29 CFR 1926.1153) requires employers to offer medical surveillance to workers who must wear a respirator for silica exposure 30 or more days a year. That exam includes a chest X-ray read by a NIOSH-certified B reader, a pulmonary-function test, and a respiratory-focused physical — the initial exam offered within 30 days of assignment and follow-ups at least every three years, all at the employer's expense.

Enforcement of the silica rule has intensified. Under the civil penalty amounts effective January 2025, a serious violation tops out at $16,550 and a willful or repeated violation at $165,514 per violation, and reporting on FY2025 activity describes rising average silica citations in construction. Whatever the exact figures on a given project, the direction is clear: documented, defensible surveillance is no longer optional paperwork.
